X-Rays:
For second opinion, legal or insurance purposes, x-ray films will be copied and the charge must be paid before release. Original films are not released by our office. Please allow three working days for film copies.

Financial Policy:
Our fees are within the customary range for this area and reflect the level of care you will receive and the level of training by your physician. Our doctors participate with most insurance plans. All applicable co--pays, deductibles and office charges will be your responsibility as directed by your insurance. You will be asked to read and sign our Financial Policy prior to treatment. Any questions regarding your insurance can by answered by our Billing Department at (570) 307-1768. Please be aware that if you belong to an HMO, we cannot see you without a referral.

Litigation:
Today, many injuries are associated with legal action. Financial responsibility to the doctor rests with the person who is injured and received treatment, NOT with the person who is being sued. Therefore, you are personally responsible for all balances due regardless of the status of any legal action. The physicians will not participate in depositions, court appearances, etc. We do not accept new patients with ongoing litigation. We do not bill your attorney. Disability Forms:
Fee is usually $5 per form, but additional fees may apply depending on the type of form. We will try to complete your forms as quickly as possible, however they may take 5-10 days to complete.
